Personal
Risk Assessment
The first part of the executive health program is the
online health risk assessment and profile. This is an assessment
of lifestyle, family history, basic demographics, and personal
history which is completed on-line. This report summarizes
the health risks, health behaviors, and medical and self-care
patterns which are needed to evaluate employee
wellness. The data focuses on health risks that are modifiable
thus allowing the individual to plan more effective corporate
wellness programming in order to better manage their health.
Results are provided in personalized reports for each executive.
A comprehensive annual health assessment provides subsequent
comparative analysis. All information is completely confidential.
Lab Work
Lab work is a vital wellness
program benefit that is performed at your work-site at a pre-arranged
time to minimize the inconvenience to the executive. Lab
work includes a complete
blood count, comprehensive
metabolic profile, complete
lipid profile, urinalysis,
thyroid panel, cardiac
risk markers, PSA (as
indicated), uric acid,
inflammatory markers,
and HgbA1C for diabetics.
Other specific tests may be added by request.
3-D Heart and Lung
Scan
This technology
integrates detector design, volume imagining capabilities
and advanced software to allow consistent imaging of the heart
and lungs. This exam is a wellness program benefit that is
noninvasive and thorough and is individually scheduled to
accommodate the executives needs. The scan will be completed
prior to the physical exam and health assessment and results
reviewed during the comprehensive consult.
Physical
Assessment
Each participant in the executive health services will
receive a full physical exam and health assessment at the
medical facility. The executive will complete: a treadmill
stress test, scan to evaluate total body composition including
bone density, percent body fat, lean body mass and hydration
status, and have a thorough physical exam and health assessment
completed by the physician. Individual appointments will be
coordinated through the marketing director personally, who
will help ensure that all preliminary testing has been completed
prior to the day of the executive health exam.
Review
of the
Personal Reports
The physician will provide a personalized report during
the one-on-one consultation with the executive at the end
of the executive health exam. The executive will receive all
of the results of the comprehensive health assessment as well
as counseling and education on how to manage existing conditions
and identified health risks to make better health decisions
for the future.
Cohort
Report
The Cohort Report is a compilation of your populations
data and summarizes many key areas of executive health and
employee wellness:
